Permanent patients

To register as a new patient either at Coed y glyn or Glan Menai you’ll need to come to the surgery with;

  • Two forms of ID
  • At least one should have a photo
  • At least one should have your local address
  • Completed GMS1 form
  • Completed health questionnaire

To complete the GMS1 form you’ll need your NHS number. If you don’t know it you’ll have to contact your previous surgery. We won’t do this on your behalf. You will also need your previous surgery’s details.

Before seeing a doctor you’ll need to have a new patient health check with the nurse.

Once you’ve registered it can take a fortnight or more for your notes to reach us from your previous surgery. We can’t get them any sooner except in emergencies.

If you want a repeat prescription we’ll need to see your repeat slip.

Temporary patients

You can register as a temporary patient if you need urgent care. You will need to come to the surgery with a completed GMS3 form.

To complete the GMS3 form you’ll need your NHS number. If you don’t know it you’ll have to contact your usual surgery. We won’t do this on your behalf. You will also need your usual surgery’s details.

If you want a repeat prescription we’ll need to see your repeat slip.
